Missouri Birth and Death Certificates
Cass County Health Department prints Birth/Death Certificates Monday through Friday 8Am – 4:30PM. CCHD can only print a Birth or Death record that has occurred in Missouri.
What you will need to obtain a copy of a Birth Certificate:
- Photo ID of the Applicant (person obtaining certificate), if no photo ID, then two forms of ID from credible sources with your name
- Cost - $15 Cash or Check made out to CCHD
- Birth Application
Birth certificates are available from 1920 to the present. Contact the State of Missouri for anything prior.
Death Certificates:
- Photo ID of the Applicant (Person obtaining certificate), if no photo ID, then two forms of ID from credible sources with your name
- Cost - $13 Cash or Check for the first copy, $10 for each additional copy
- Death Application
Death certificates are available from 1980 to the present. Contact the State of Missouri for anything prior.
