WIC (Women, Infant and Child) Services
Women, Infants and Children is a supplemental program that serves pregnant women, non-breastfeeding postpartum women (up to 6 months after delivery or termination of the pregnancy), breastfeeding women (up to 1 year after delivery as long as they are breastfeeding the baby), infants from birth up to 1 year of age, and children up to their 5th birthday, based on nutritional risk and income eligibility.
How do I qualify for WIC?
Clients must meet the WIC income Guidelines and have a nutritional risk

What forms should I bring to my Appointment?
Nutritional Assessment for Infants: To be filled out for an infant (0-12 Months of age) and brought to the Certification or Re-Certification appointment. Nutritional Assessment for Infants
Nutritional Assessment for Children: To be filled out for a child (12 Months and 1 Day to 5 years of age) and brought to the Certification or Re-Certification appointment. Nutritional Assessment for Children
Nutritional Assessment for Women: To be filled out for a woman and brought to the Certification or Re-Certification appointment. Nutritional Assessment for Women
Special Formula Form: To be filled out by the infant’s doctor, if they need to be on a special formula. Special Formula Form
The person who is being certified must be present
Proof of the following MUST be provided for all clients applying or reapplying for WIC:
Identity: Adults should bring a photo ID such as a driver’s license or ID card (work, school, or state) or a passport. Infants and Children; a birth certificate, immunization record, hospital document or insurance card will be required.
Residency: A current utility bill, rent or mortgage receipt, or a written statement from a third party.
Income: A recent paystub, the previous year’s federal tax form, and any other income source documentation are needed.
